The authors demonstrated that high organic food intake was associated with a lower overall risk of cancer. We wish to commend the authors for their meticulous and robust effort. However, there are a few pertinent issues presented in the study that carry important policy and research implications.An interesting finding was that combining a high frequency of organic food consumption and a high-quality diet was not associated with a reduced risk of overall cancer (eTable 6 in the Supplement). This is in fact supported by eTable 3, which shows that an increased modified Programme National Nutrition Santé Guideline Score (when analyzed as a continuous variable) is significantly associated with an increased risk of cancer (hazard ratio, 1.005; 95% CI, 1.001-1.009). These findings are drastically different from what one would expect. Even though the authors hypothesized that the higher intake of pesticide-contaminated products may counterbalance the benefits of high-quality foods, this is again contradictory to Table 1, in which participants with the highest modified Programme National Nutrition Santé Guideline Score also had the highest organic food score. Hence, pesticide exposure in this particular group should theoretically be the lowest. Furthermore, contrary to the authors’ hypothesis, the European Food Safety Authority reported that the mean amount of single pesticides was similar for both conventional and organic food products.
